We want to hear from you!Thank you for participating in the Canadian Cochrane Centre's What's your story? competition.We know the evidence found in Cochrane Reviews is used every day by health practitioners, consumers/patients, carers, and policy-makers all over the world to make health treatment and health system decisions. We want to hear how you've used Cochrane evidence to make your decisions, whether they be clinical, personal, policy-related, or involving any other area. Hearing how Cochrane evidence has affected your health and healthcare decisions, in turn, gives us powerful evidence to show our supporters and funders that the work of The Cochrane Collaboration in Canada is making a difference in the lives of Canadians. We're also interested in hearing how Cochrane training or involvement has affected your career or professional development. Stories and quotes you submit will be used throughout our website, annual report, funder reports and various other promotional materials to provide an overall basis of the support Cochrane Canada has and the impact Cochrane Review evidence has on various levels of health care all over the country.Participate for Prizes!Not only will you be helping us by submitting your stories, but each participant will be awarded a $5 Starbucks gift certificate as our thanks to you. We will also award a third prize of a $250 Amazon.ca gift certificate, a second prize of a $500 Amazon.ca gift certificate, and a grand prize of an iPad to the authors of the stories selected as the top three submissions.
Participants are allowed and encouraged to submit more than one story.Submissions will be accepted in both French and English. Contest dates:The contest is open from 9 a.m. EDT, Thursday 24 April 2014 to 4 p.m. EDT, Thursday 31 July 2014. Please note:Submissions may be used by the Canadian Cochrane Centre, as stated above, in full or part, whether or not the participant’s story has been chosen for a prize. You will be asked for permission to use your submission in the survey. Submissions may be edited for spelling, grammar, language composition, and length. Please be advised that we are collecting address information only to take note of general locations for statistical purposes and to award prizes. Questions denoted with a red * are mandatory.
